Part-Time Social Worker (Exempt) – 16 bed Skilled Nursing Facility

24 Hours per Week | Day Shift | On-Site

What You'll Do

As our Skilled Nursing Facility Social Worker, you'll play a vital role in supporting residents and families throughout their healthcare journey by:


  • Conducting comprehensive psychosocial assessments for new and existing residents
  • Advocating for residents' emotional well-being, dignity, and rights
  • Coordinating interdisciplinary care plans and participating in resident care conferences
  • Providing counseling, support, and crisis intervention to residents and families
  • Assisting with discharge planning and coordinating post-discharge services
  • Connecting residents and families with community resources and support programs
  • Facilitating family meetings and helping resolve concerns and conflicts
  • Collaborating with physicians, nursing staff, therapy, admissions, and community partners
  • Completing MDS documentation and maintaining accurate medical records in compliance with state and federal regulations
  • Coordinating admissions, completing admission paperwork, and serving as a liaison with hospital discharge planners
  • Participating in regulatory surveys and maintaining HIPAA compliance


What We're Looking For

Required Qualifications:

  • Licensed Certified Social Worker (LCSW) or Licensed Bachelor Social Worker (LBSW) in the State of Maryland
  • Minimum of two (2) years of experience in healthcare, skilled nursing, rehabilitation, or long-term care
  • Experience with discharge planning, care coordination, and interdisciplinary team collaboration
  • Knowledge of Medicare, MDS documentation, and long-term care regulations preferred
  • Excellent communication, conflict resolution, and organizational skills
  • Compassionate, resident-centered approach with the ability to build trusting relationships
