Job description

Wells Health is hiring a Hybrid Personal Care Aide to support seniors inside a Assited Living Facility in New York.

This role is in-person
You will be the on-site Wells representative who welcomes residents, helps them get started with services, and checks in with them weekly to ensure they are doing well and staying connected to care.

Responsibilities

  • Meet new residents and introduce them to Wells Health

  • Collect consent from residents

  • Schedule and coordinate telehealth appointments

  • Assist residents during telehealth sessions when needed

  • Complete weekly wellness check-ins with every assigned resident

  • Track changes in mood, engagement, attendance, safety, or family involvement

  • Report clinical or behavioral concerns to Wells the same day

  • Maintain strong working relationships with facility staff

What success looks like

  • Reliable weekly check-ins with every assigned resident

  • Residents understand and attend their scheduled sessions

  • Issues are escalated quickly

  • Documentation is accurate and completed the same day

Requirements

  • Experience working with seniors / Sub-Acute Rehab strongly preferred

  • CNA License and/or PCA Certificate

  • CNA / social services / activities / admissions / case management background is useful

  • Strong communication and relationship-building skills

  • Highly organized and dependable, this role affects resident safety and support
