Clinical Pharmacist – Hospital (Evenings, 6 On / 8 Off)Mercy Hospital | $6,000 Sign-On Bonus

Mercy Hospital is seeking a dedicated Clinical Pharmacist to join our team on a 6 on / 8 off evening block schedule. This role plays a vital part in delivering safe, effective medication therapy and serving as a trusted clinical resource for patients and the healthcare team.

Position Overview

The Clinical Pharmacist collaborates with physicians, nurses, and other healthcare professionals to optimize patient outcomes through evidence-based medication management. This role ensures safe medication use, provides drug information, and supports Mercy’s mission, values, and Service Standards.

Qualifications

Education

  • Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy

Licensure

  • Current pharmacist license in the state of practice
Key ResponsibilitiesClinical & Patient Care
  • Review patient records to assess appropriateness of medication therapy
  • Evaluate patient conditions to ensure all health issues are properly treated
  • Develop and implement safe, effective medication plans
  • Monitor drug therapy and intervene when appropriate
  • Assess outcomes and adjust treatments as needed
  • Provide counseling to patients on proper medication use
  • Collaborate with physicians and nurses on medication selection, dosing, and therapy plans
  • Identify and prevent adverse drug events, interactions, and contraindications
Medication Management & Operations
  • Evaluate medication orders for clinical accuracy and operational considerations
  • Ensure compliance with formulary guidelines and care pathways
  • Maintain accurate documentation of patient care activities
  • Participate in medication use evaluations (MUE) and adverse drug reaction reporting
  • Perform required inspections, expiration checks, and floor stock reviews
Collaboration & Education
  • Serve as a drug information resource for medical, nursing, and pharmacy staff
  • Precept pharmacy students and contribute to staff education initiatives
  • Present in-services and educational programs
  • Support competency development within the department
  • Participate in committees and departmental initiatives
Quality & Compliance
  • Adhere to safety, confidentiality, and regulatory standards
  • Participate in quality assurance and performance improvement activities
  • Maintain current knowledge of policies, procedures, and clinical best practices
  • Complete required safety training and continuing education
Technology & Systems
  • Utilize pharmacy systems for order verification and clinical documentation (e.g., Epic)
  • Use drug information resources (e.g., Micromedex)
  • Operate pharmacy automation systems (Omnicell, McKesson, ScriptPro, etc.)

  • Maintain strong working relationships with interdisciplinary teams
