What You'll Do

  • Perform comprehensive in-home nursing assessments for home health patients
  • Develop and manage individualized plans of care in collaboration with physicians, caregivers, and the interdisciplinary care team
  • Provide skilled nursing services including medication management, wound care, disease management, and patient education
  • Complete OASIS assessments, recertifications, case conferences, and timely clinical documentation
  • Identify the need for additional home health services, disciplines, and durable medical equipment (DME)
  • Assess home environments to promote patient safety, fall prevention, and improved outcomes
  • Obtain and document physician orders, including verbal orders, per agency and regulatory guidelines
  • Educate patients and caregivers on care plans, medications, and symptom management
  • Maintain compliance with CMS, Medicare, and home health regulations
  • Serve as a preceptor or mentor to other nursing staff as needed

Qualifications

  • Graduate of an accredited nursing program
  • Current, active California Registered Nurse (RN) license and CPR certification

  • Strong interpersonal and communication skills
