Serving 10 counties: Cook, Will, DuPage, Kane, Kendall, McHenry, Lake, Ogle, DeKalb, LaSalle

What You'll Do

  • Assess patient conditions at each visit and document findings
  • Complete evaluations: vitals, medication review, OASIS assessments, wound care, IV therapy
  • Manage and monitor wound healing
