Job description

Part-time Registered Nurse Case Manager Anticipated Territory: Sacramento County Schedule: Every Saturday and Sunday Full Days, or Saturday/Sunday plus 3 weekdays as needed. Please note: All opportunities at 21HHS require field visits to patients' homes.

What you'll do

Conduct Start of Care (SOC) admissions and OASIS assessments. Perform followโ€up, evaluation visits, and home visits within the designated territory. Reconcile medications, provide wound care, and perform treatments and procedures. Monitor clients' conditions and report changes to the clinical team. Collaborate with physicians and agency staff to deliver comprehensive care. Initiate and administer IV medication infusions (antibiotics, diuretics) following established protocols.
