Client is a 19-year old male with TBI, wheelchair bound, limited speech, hemiplegic

Available Shifts: 

  • Park City:  Every day of the week, 12 hour shifts days and nights, 7-7

We are looking for an RN or LPN who is interested in the following:

  • Medication set up and oversight
  • Provide thoughtful care to clients based on personalized Plan of Care
  • Provide in home assessments
  • Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
  • Collaborate with client's family on plan of care, health changes, and concerns
  • Weekdays include going to school with client
  • Total personal cares
