RN Charge Nurse - Evenings Full Time or Part Time
Rate of Pay: $42.00 Base Rate. 8% shift differential (must work minimum 4 hours): weekdays $45.36, $46.20 with 2% weekend differential
Lutheran Center at Poughkeepsie has an immediate need for an RN Charge Nurse.
We are looking for a qualified charge nurse to be responsible for the physical assessments of all residents, and will assist with the evaluation or assessment of residents with a change of condition. The Charge RN provides leadership to the other nursing staff on the unit. The ideal incumbent will possess both strong clinical and managerial skills. A high degree of empathy and interpersonal awareness is critical, as is the ability to take initiative and act in accordance with The Lutheran Care Center philosophies and standards.

Responsibilities
  • Directs and supervises the activities of all nursing personnel on designated shift through assignment of duties based on resident need and staff mix.
  • Carries out established policies and procedures, medication and treatment administration, performs and/or supervises and assists with resident care as needed.
  • Review residentsโ€™ condition and needs daily and provides clinical updates to LPNs and CNAs on an ongoing basis. Notifies physician/family of changes in resident condition.
  • Observes nursing care and performs daily rounds to ensure that nursing care is carried out as directed and treatment is administered in accordance with established policies/procedures.
  • Makes recommendation for additional or modified services, in accordance with residentsโ€™ total needs.
  • Assist new residents in adjusting to their new environment and informs them of applicable policies and procedures.
  • Observes the nutritional needs of residents, by actively participating at mealtimes. Notifies dietary department changes in diets, etc.
  • Accompanies physician on rounds to answer questions, assist with examinations and treatments. Initiates doctorโ€™s orders as necessary. Administers and forwards medication orders to pharmacy.
  • Investigates and receives complaints. Refers complaints to Clinical Manager as necessary.
  • Develops, implements, and updates nursing care plan to assure continuity of residentโ€™s needs.
  • Maintains resident medical and nursing records. Enters nursing observations and actions taken as per policies and procedures.
  • Provides orientation and assists new personnel in nursing care techniques and procedures. Facilitates attendance of staff at in-services and committee meetings.
  • Investigates, reports, and records all accidents of residents and staff, visitors to Clinical Manager, physician and family.
  • Operates or supervises the operation of equipment assigned to unit. Provides assistance and guidance to nursing team as required and initiates maintenance repair as indicated.
  • Assists physician with monthly review of medication and physical examinations for each resident. Prepares lab work slips and writes weekly and monthly summaries of patientโ€™s condition.
  • Maintains appropriate records including PRI forms, documentation needed for PRIโ€™s, and 24-hour shift report. Participates in completion of MDS+.
  • Gives report to on-coming nurse on conditions of residents and/or any unusual actions taken.
  • Participates in, or provides staff for, meetings to discuss resident care needs. Submits minutes to Clinical Manager on a monthly basis. Participates in in-service education.
  • Attends meetings of supervisory and administrative staff to discuss units operation, staff training needs. Helps to formulate programs to assure quality level of unit care.
Experience and Skills
  • Valid NYS RN license required. NEW GRADUATES WELCOME!
  • Associates Degree in Nursing with Bachelors in Nursing preferred.
  • 3-5 years experience in an environment with clinically complex cases, with preference given to those with experience in long term care. Willing to train the right candidate
