Position Summary
Ideal Home Health Agency is seeking a compassionate, dependable, and organized Part-Time Registered Nurse (RN) to join our clinical team. This position is ideal for an RN seeking a flexible schedule while making a meaningful impact on patients' lives.
The Part-Time RN is responsible for conducting Start of Care (Admissions), Recertification, Supervisory, and other required skilled nursing visits, while providing clinical oversight and supervision of Home Health Aides. The RN works closely with physicians, caregivers, office staff, and interdisciplinary team members to ensure patients receive safe, high-quality care that complies with agency policies and state and federal regulations.
Key Responsibilities
  • Complete comprehensive Start of Care (Admission) assessments for new patients.
  • Perform Recertification visits and complete all required OASIS and clinical documentation accurately and within required timeframes.
  • Conduct Home Health Aide Supervisory Visits to evaluate aide performance, ensure quality of care, and provide coaching and guidance as needed.
  • Develop, review, and update individualized Plans of Care in collaboration with physicians and the interdisciplinary team.
  • Assess patients' physical, emotional, psychosocial, and environmental needs during each visit.
  • Monitor patient progress and promptly communicate significant changes in condition to the physician and appropriate team members.
  • Provide education to patients and caregivers regarding disease management, medications, safety, infection prevention, and available community resources.
  • Ensure all documentation complies with Medicare, Medicaid, ACHC, and agency standards.
  • Collaborate with scheduling, intake, therapists, social workers, and office staff to coordinate patient care.
  • Participate in quality improvement initiatives, chart audits, and compliance activities as assigned.
  • Maintain patient confidentiality and comply with HIPAA and all applicable state and federal regulations.
  • Attend required meetings, trainings, and continuing education programs.
Qualifications
  • Current, active, and unrestricted Pennsylvania Registered Nurse (RN) license.
  • Minimum of one (1) year of nursing experience required; home health experience preferred.
  • Experience with admissions, OASIS documentation, or home health assessments is preferred but not required.
  • Strong assessment, clinical judgment, and documentation skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and effectively manage a flexible schedule.
  • Reliable transportation, valid driver's license, and willingness to travel within assigned service areas.
  • Current CPR certification.
  • Ability to pass all required background checks, health screenings, and pre-employment requirements.
