HOW YOU'LL CONTRIBUTE The Licensed Practical Nurse (LPN) Licensed Vocational Nurse (LVN) provides direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient's needs.   • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.

WHAT WE'RE LOOKING FOR: This role requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Graduate of an accredited Licensed Practical/Vocational Nurse program. Experience: Minimum six months of rehab experience and acute care setting preferred. License: Current state licensure as a Licensed Practical/Vocational Nurse. Certifications: CPR/BLS certification required; IV Certification highly preferred or must be obtained within 6 months.
