Summary of Job Duties:
The Registered Nurse will promote wellness; improve outcomes, and self-care within a defined population of patients. The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and
implement individual health plans and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes.
Qualifications:
Minimum Qualifications:
  • Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing ;
  • Valid RN License;
  • Minimum of 1 year Registered Nursing experience.

Preferred Qualifications:
  • 3 years of RN experience with current BLS certification.
  • Knowledge or experience in PCMH
  • ACLS certification or obtain certification within one year of hire date.

Additional Information:
BENEFITS ELIGIBLE - This position offers part-time hours ranging from 20 to 30 hours per week, with a schedule that includes daytime shifts. There are no night, weekend, or most State holiday commitments associated with this role.
Responsibilities:
  • Collects and documents assessment data from patients and/or others per UAMS standard.
  • Develops patient's Plan of Care (POC) per department/unit standards utilizing assessment data.
  • Evaluates the progress of patients toward attainment of desired outcomes.
  • Administers medication following department/unit standards and the Nurse Practice Act.
  • Verifies and evaluates care delivered to ensure it is consistent with patient and family-centered care principles, practice standards, and regulatory requirements.
  • Supports the integration of current evidence into nursing practices.
  • Promotes an environment conducive to positive educational experience for all students and staff.
  • Provides effective patient/family education/discharge planning.
  • Effectively collaborates and communicates with nursing staff at all levels, interdisciplinary teams, executive leaders, and other stakeholders.
  • Communicates care appropriately through documentation in the medical record.
  • Reports variances in care/ systems/ processes through the appropriate means.
  • Participates in the recruitment/retention of staff.
  • Facilitates the development and implementation of nursing practice standards that are evidence-based and in compliance with ANA standards.
  • May perform other duties as assigned.
