As an Occupational Therapist with Premier Home Health Providers, you will help patients regain independence, improve function, and safely navigate daily life - all from the comfort of their own home.

Please note that this role is per diem with a rate of $110 - $130 per visit.
Responsibilities
  • Evaluate patients' functional abilities and home environments to identify needs and safety concerns
  • Develop personalized treatment plans and communicate them to physicians and the care team
  • Teach and retrain patients in daily living skills affected by illness or injury
  • Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
  • Maintain accurate clinical records and promptly report changes in patient condition
  • Coordinate with the broader care team to ensure seamless, patient-centered care
  • Educate family members and caregivers on the patient's treatment approach
  • Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
  • Support discharge planning as patients transition out of home health care
  • Perform other duties as required for patient services

Requirements
  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
  • Registered with the National Registration Examination of the American Occupational Therapy Association
  • Active California Occupational Therapy license
  • Minimum 2 years of clinical OT experience
  • Current CPR certification with AHA
  • Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
  • Strong communication and organizational skills
