Job description

Personal Home Care of North Carolina (PHC) is a rapidly growing full service Home Care Agency looking for energetic and dedicated skilled caregivers that can fulfill the clinical needs of our growing patient population.

We are growing quickly and looking for dedicated Home Care Nurse to provide excellent patient care and employee supervision and training.

Position: Part-time, as needed Mon-Sun

Service Area: Cary/Raleigh NC

Home Care Registered Nurse is responsible to perform general nursing duties for patients under the direction of a Physician at patient’s home.

Home Care RN Job Functions and Schedule will vary, but will generally include:

  • Provision of nursing care; including assessments, treatments, diagnostic procedures, and patient education based on plan of care and physician’s orders.
  • Observe signs and symptoms, reactions to treatment, drug response/interactions and changes in the patient’s condition; with appropriate follow up in care and reporting to supervisor or physician as needed
  • Coordinates care, creating plans of care for in home aides as needed with patient/responsible party, physician, other disciplines and or internal and external resources.
  • Documents accurately, objectively and timely in accordance with regulations and agency policy/procedures
  • Performs Supervisory visits for patients and in home aides.
  • Performs Training and Orientation for all newly hired in home aides annually and at hire.
  • Participates in multi-disciplinary team meetings weekly.
  • Participates in learning and job-enhancement activities as directed by the supervisor.

Home Care RN Job/Qualification Requirements

  • Prior Home Care Experience or Educator experience is a plus (2+ years)
  • A positive attitude

  • Excellent time management, organizational and reporting skills
  • Strong communication skills, both written and verbal
  • Excellent planning, organizational and reporting skills
  • Bi-Lingual (Russian/English; Spanish/English) is a plus
  • Pediatric experience is a plus
  • Current NC or multi-State RN License
  • Current Drivers License and Automobile liability insurance issued in NC
