We're currently seeking a Full-Time Registered Nurse (RN) to join our ever-expanding Home Health team in the Lititz/Ephrata , PA and surrounding areas. This role offers the chance to directly provide patient care within homes, truly making a difference in the lives of our patients. Opportunity for growth into a Full-Time position.

What You'll Do:
  • Conducting Start of Care comprehensive assessments and completing OASIS timely
  • Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
  • Preparing accurate, orderly, and timely documentation
  • Coordinating care with patient and provide proper reports to Case Manager
  • Provides health care instructions to the patient/family as appropriate per assessment and plan

About You:
An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
