Job description

  • Position Summary
    • Responsible for navigation and coordination of oncology patients in oncology private practice. Assures coordinated services to meet patient needs utilizing a multi disciplinary team and community resources. Maintains ASCO & ONS standards, regulatory and accreditation standards in daily practice and mentors other teams members for compliance. Reviews physician orders for completeness and compliance for CPOE, relevant standards and requirements. Responsible for assisting with authorizations for treatment including oral chemotherapy, imaging and pathology services. Maintains conflict/complaint resolution within practice and as per system policies.

  • Core Job Functions
    • Follows national guidelines and best practices to navigate cancer patients along a disease continuum of care. Facilitate communications between the managing physicians, patients and diagnostic and treatment facilities. Facilitate continuum of care navigation by scheduling and assuring patient compliance with treatment plan.
    • Provides a comprehensive review of patients to determine and address barriers to care. Addresses barriers that range from psychosocial and educational to transportation and financial. Facilitates between the various departments associated with the patients' care continuum to assure barrier issues are resolved to the best of SJC's resources.
    • Performs assessment and comprehensive review of history on new patients and established patients. Measures and documents vitals signs. Performs medicine reconciliation.
    • Reinforce physician's advice to patients regarding reactions to treatment and care. Involves patient and family in planning process. Identifies learning barriers, readiness to learn and cultural issues that can impact learning for patient and family.
    • Manage provider based authorizations by coordinating with the insurance representative and acquiring the pre-authorization according to protocol. Maintain accurate electronic documentation in support of the authorization services processes.
    • Open communication with patients/families is utilized to achieve positive patient/family outcomes. Follow up on patient/family/customer complaints in a timely manner.
