Job description

RN Home Health Clinical Manager

Always Best Care Home Health is seeking a RN Clinical Manager with Medicare experience to guide, lead and build a fast growing, results oriented Home Health Agency. The Clinical Manager compliments the Administrator in planning, developing and supervising the activities of Licensed and non-licensed personnel who provide health care and nursing services to Home Health patients. This position assists with the development and implementation of nursing services, objectives, policies and procedures in accordance with Federal and State regulations.

Additional responsibilities:

Clinical Manager will be responsible for coordinating, organizing, implementing and evaluating the patient care delivery system, ensuring that physician orders are completed, timely and as ordered. Will monitor clinical documentation for completeness and accuracy. Provide education to clinical staff as well as patient/families as needed. Responsible to meet with physician, social worker, speech, PT,OT and clinical staff to monitor status of patient, effectiveness of patient care plan, appropriateness and continued appropriateness for home health. Identify issues and direct changes. Maintain communication with all disciplines to ensure that efficient, cost effective, appropriate care is delivered. Data collection, Auditing, Reporting, Interdisciplinary meetings. Monitor financial data to ensure that office is operating at peak efficiency, such as staffing levels, patient supply needs, insurance claims and reimbursement. Must have excellent, organizational skills, ability to critical think and problem solve, effective communication and multitasking skills. Working knowledge of EMR, and Oasis, Hiring, Evaluations, Counseling, Termination, Education and orientation of clinical staff.

Qualifications:

  • RN with current license to practice in California.
  • Minimum of Five(5) years nursing experience with preference for at least one year in home care.
  • Knowledge of community resources, Medicare and state regulations, ACHC requirements, and reimbursement criteria for third party and private payers.
  • Excellent communication skills (verbal and written), interpersonal skills, and documentation skills.
  • Demonstrated organizational and time management skills, critical thinking, and ability to work independently.
  • Computer data entry skills.

Job Requirements

Ideal candidate will have at least 3 -5 years of experience in a Medicare Home Health environment and hold a current RN license in California. Ideal candidate will also have a proven track record of leading and managing teams with at least 2-3 years in a leadership role, plus have an outstanding desire to provide great customer service and creative clinical management.
