Your future role at a glance

Location: Jacksonville, FL 

Facility: Ascension St. Vincent's Riverside Hospital 

Schedule: Day shift | Part-time | Saturday and Sunday 8 a.m. - 4:30 p.m., plus one weekday shift

How you'll make an impact in this role
  • Orchestrate the full interdisciplinary care plan from admission through discharge.

  • Design continuum-of-care plans that integrate patient and family values.

  • Monitor clinical progress through continuous assessments and record reviews.

  • Advocate for essential resources and eliminate barriers to patient care.

  • Serve as a clinical resource for documentation accuracy and post-discharge options.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
