Advanced Practice Provider (APP) โ€“ Rheumatology

Provide compassionate, specialized care to patients living with autoimmune and rheumatologic conditions.

Raleigh Medical Group is seeking a dedicated and patient-focused Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to join our Rheumatology team. This position provides comprehensive specialty care for patients with autoimmune and inflammatory disorders while working collaboratively with physicians, clinical staff, and other healthcare professionals to deliver exceptional patient outcomes.

The APP plays an integral role in evaluating, diagnosing, treating, and educating patients while promoting continuity of care through evidence-based treatment plans, chronic disease management, and patient-centered care.


Key Responsibilities
  • Evaluate, diagnose, and manage patients with rheumatologic and autoimmune diseases, including inflammatory arthritis, connective tissue diseases, gout, osteoporosis, and related conditions.
  • Order, interpret, and review laboratory studies, diagnostic imaging, and other diagnostic tests.
  • Develop and implement individualized treatment plans using evidence-based clinical guidelines.
  • Prescribe and manage medications, including disease-modifying antirheumatic drugs (DMARDs), biologic therapies, corticosteroids, and other specialty medications in accordance with applicable laws and organizational policies.
  • Administers medications and treatments as directed within scope of certification
  • Documents patient assessments, interventions, medications, laboratory results, and patient responses accurately in the electronic health record (EHR)
  • Processes non-emergent patient calls and communicates provider-guided clinical information appropriately
  • Coordinates laboratory testing, imaging studies, referrals, infusion appointments, and specialty consultations
  • Monitor disease activity, medication effectiveness, laboratory results, and patient response to treatment.
  • Provide patient education regarding disease processes, medications, treatment options, lifestyle modifications, and self-management strategies.
  • Coordinate care with referring providers, specialists, infusion services, imaging centers, and other members of the healthcare team.
  • Document patient encounters, diagnoses, treatment plans, medications, laboratory results, and follow-up recommendations accurately in the electronic health record (EHR).
  • Respond to patient messages, triage clinical concerns, and provide continuity of care between office visits.
  • Collaborate with physicians and the interdisciplinary care team to deliver high-quality, patient-centered care.
  • Participate in quality improvement initiatives, peer review activities, continuing education, and clinical guideline development.
  • Perform additional duties as assigned.
ย Qualifications

Required:
ย 

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • Current North Carolina licensure appropriate to the profession.
  • National certification appropriate to the profession.
  • Current DEA registration, if required.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical assessment, diagnostic reasoning, and critical thinking skills.
  • Experience using electronic health record (EHR) systems.
  • Excellent communication, organizational, and interpersonal skills.

Preferred:

  • One or more years of Rheumatology, Internal Medicine, Family Medicine, or other specialty practice experience.
  • Experience prescribing and monitoring biologic therapies and immunosuppressive medications.
  • Current Advanced Cardiac Life Support (ACLS) certification.
Physical Requirements
  • Alternates between sitting, standing, and walking throughout the workday.
  • Periods of bending, reaching, and fine motor activity.
  • Occasionally lifts or carries items weighing up to 10 pounds.
  • Corrected vision and hearing within the normal range.
  • Ability to work occasional extended or irregular hours, as needed.
