Full and Part Time positions are available:

AM (6:00am-2:30pm) PM (2:00pm-10:30pm) NOC’s (10:00pm-6:30am)

All scheduled positions require an every other weekend and every other holiday rotation.


  • Direct the day-today functions of the nursing assistant
  • Receive physician's orders and execute as appropriate
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided to each resident in EMR
  • Ensure appropriate documentation and communication of changes in a resident's needs and/or condition
  • Prepare and administer medications as ordered by the physician
  • Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician
  • Consult with the resident's physician in providing the resident's care and treatment
  • Review the resident's EMR for specific treatments, medication orders, diets, etc.
  • Maintain established nursing standards
  • Make periodic checks to ensure that prescribed treatments are being properly administered by certified nursing assistants and to evaluate the resident's physical and emotional status
  • Notify the resident's physician and P.O.A./Guardian of changes in condition
  • Administer professional services such as; catheterization, tube feedings, wound care, etc.
  • Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes


Requirements:
  • Possess a current, active license to practice as an LPN or RN in the State of WI
  • Understands and supports Pine Haven’s Mission, Vision and Values and Resident-Centered Engagement Principles
  • Regular attendance, including timely arrival at work, to support positive working relationships with fellow team members and to support quality resident care
  • Ability to work in a fast paced rapidly changing environment
  • Attention-to-detail and organizational skills required
  • Strong teamwork and communication skills are critical
  • Ability to lift up to 40 pounds
