Position Overview

Big Leap Family Guidance Centers is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our team in a full-time or part-time remote capacity. Key Responsibilities

• Conduct thorough patient intakes focusing on diagnostic assessments and functional evaluations to inform treatment goals and plans.
• Provide brief, focused interventions for patients requiring mental health support, offering an immediate safety net while promoting long-term wellness.
• Collaborate with primary care providers to develop treatment plans, advising them on patient transitions between primary care and specialty mental health services.
• Monitor patient progress through regular follow-ups, adapting treatment strategies as necessary to support optimal outcomes.
• Assist in the creation of relapse prevention plans to empower patients in maintaining stability and achieving their personal health goals.
• Engage in community outreach efforts to identify at-risk populations and develop strategies to prevent deterioration of mental health and wellbeing.
• Document all patient interactions and progress meticulously in electronic health records systems, ensuring compliance with all regulatory and organizational standards.
• Coordinate with external providers, when required and authorized by the patient, to ensure continuity of care across treatment paradigms.
• Provide educational resources and coaching to empower patients to actively participate in their recovery and self-management.
• Consult with clinic management and peers on behavioral health program evaluation and optimal service delivery models.
• Support patients in adhering to medical recommendations from their primary care providers, including providing strategies for managing medication side effects.

Patient Population / Client Focus

The ideal candidate will engage a broad spectrum of clients presenting with varying mental health needs, from anxiety and depression to more complex psychosocial challenges. Your role will involve understanding the unique cultural and social factors that influence each patient's health and wellbeing, ensuring that care is personalized and effective.

Qualifications

• Master's degree in Clinical Counseling or a related field is required.
• Current LPC (Licensed Professional Counselor) license with an established record of ethical practice.
• Extensive knowledge of mental health assessment, treatment planning, and therapeutic modalities, with a focus on evidence-based practices.
• Proficiency in conducting psychodiagnostic evaluations and utilizing psychotherapeutic techniques to foster positive behavioral changes.
• Ability to maintain clinical records accurately and prepare comprehensive reports.
• Demonstrated capacity to establish and nurture effective relationships with patients, families, and multidisciplinary team members.
• Strong analytical skills to evaluate patient needs and adapt services accordingly.

Preferred Experience

• Experience in a community health or integrated care setting is preferred.
• Fluency in both English and Spanish is advantageous, as it will help broaden access to services for a diverse patient community.
• Demonstrated experience working collaboratively in an interdisciplinary team context.

Compensation and Benefits

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
