What You'll Do - Coordinate Care: Schedule field visits, prioritize nursing assignments, and ensure all referrals and admits are processed efficiently.
- Operational Excellence: Manage the weekend log, run compliance reports, and facilitate DME (Durable Medical Equipment) orders to ensure patients have what they need, when they need it.
- Clinical Collaboration: Work closely with RNs, Physicians, and the IDT to update patient findings and manage documentation such as DNRs and triplicates.
- Patient Advocacy: Act as a key point of contact for families, providing information, education, and addressing concerns with empathy and professionalism.

Qualifications, Skills, and Abilities:- Education: Graduate of an accredited school of vocational nursing.
- Licensure: Current State license as an LVN and a valid State Driver's License.
- Experience: Minimum of two years of experience as an LVN in a clinical setting; hospice or home health experience is strongly preferred.
- Skills: Strong computer proficiency, excellent interpersonal communication, and top-tier organizational abilities.
- Requirements: Reliable transportation and current auto liability insurance; must be able to pass a criminal background check.
Work Environment - This position involves a hybrid schedule, with duties performed both remotely and in the office.
- The role requires long periods of sitting while conducting computer work.
- You will spend a significant amount of time on the phone and in front of computer screens to manage branch communications and data.
Physical and Mental Effort - Mental Resilience: You must be able to work under stressful conditions to meet tight deadlines and urgent patient needs.
- Quick Decision-Making: This role requires the ability to make rapid decisions and efficiently acquire necessary resources.
