Job Summary:

Carease Health - MA is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of healthcare professionals dedicated to reducing hospitalizations and improving patient care. As a key member of our staff, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. With a focus on building strong relationships, maintaining a patient-centered approach, and being a collaborative team player, you will be an integral part of our mission to improve the lives of nursing home patients.

Responsibilities:

  • Provide high-quality preventative care to residents in skilled nursing facilities, including on-site visits and remote work from home

  • Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) technologies to enhance patient care

  • Maintain accurate and timely documentation, supported by a dedicated scribe team

  • Build strong relationships with facility staff, patients, and family members

  • Remain up-to-date with industry developments, best practices, and evidence-based guidelines

  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and Documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Masters-level education in a relevant field
