Licensed Practical Nurse (LPN) / Registered Nurse (RN)

Vinculum Care – Macomb, MI

Vinculum Care, a locally owned home health agency serving the Port Huron area since 2019, is seeking a compassionate, dependable, and professional Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our growing team as we expand into Macomb County. 

We are committed to providing high-quality, person-centered care to individuals in their homes and throughout the community.


Position Overview

This position is responsible for providing skilled nursing services, conducting client assessments, developing and monitoring care plans, and ensuring clients receive quality care in accordance with physician orders and agency policies.

Both full-time and part-time opportunities are available.


Responsibilities

  • Perform nursing assessments and reassessments of clients.
  • Develop, implement, and monitor individualized care plans.
  • Administer medications and treatments as prescribed.
  • Coordinate care with physicians, caregivers, and other healthcare professionals.
  • Maintain accurate and timely clinical documentation.
  • Monitor client health status and report significant changes.
  • Provide education and support to clients and their families.
  • Ensure compliance with state regulations, agency policies, and professional nursing standards.
  • Participate in care conferences and interdisciplinary team meetings as needed.


Required Qualifications

  • Current and unrestricted Michigan LPN or RN license.
  • Graduate of an accredited Practical Nursing or Registered Nursing program.
  • Current CPR/BLS certification.
  • Valid driver's license and reliable transportation.
  • Ability to pass a background check and drug screening.
  • Strong assessment, communication, and organizational skills.
  • Ability to work independently while maintaining professional standards of care.
  • Proficiency with electronic documentation and basic computer skills.


Preferred Qualifications

  • Home Health, Hospice, Private Duty, Case Management, or Community Health experience.
  • Experience conducting client assessments and care planning.
  • Knowledge of Medicaid, Medicare, and home care regulations.
  • Experience supervising caregivers or direct care staff.
