Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients' homes, helping them regain strength, mobility, and confidence in their daily lives.


Key Responsibilities
  • Conduct in-home evaluations, start of care, and follow-up visits
  • Develop and implement individualized treatment plans
  • Improve strength, balance, mobility, and functional independence
  • Educate patients and caregivers on safety and equipment use
  • Document visits using WellSky Kinnser EMR
  • Collaborate with physicians, RNs, and the care team

Qualifications
  • Active Arizona Physical Therapist license
  • CPR certification (AHA)
  • Minimum 1 year of clinical experience (home health preferred)
  • Strong clinical judgment and time management
  • Reliable transportation, valid driver's license, and insurance
