Job Summary
This position supports Christus Health and provides community support services for team members throughout southeastern Texas and southwestern Louisiana. The Wellness Coordinator connects teammates with resources and assistance programs that address social determinants of health, including housing, food access, legal services, mental health support, and financial assistance.
Travel: Up to 60% throughout southeastern Texas and southwestern Louisiana.
Essential Responsibilities
  • Conduct and analyze needs assessments to identify team member needs and connect them with appropriate community resources.
  • Assist team members with applications for assistance programs and benefit enrollment.
  • Coordinate with emergency assistance funds to support teammates facing financial hardships.
  • Build and maintain relationships with community organizations and service providers.
  • Partner with department managers to ensure team members receive appropriate support.
  • Facilitate educational sessions focused on whole-person health and wellness.
  • Conduct 30-, 60-, 90-day, and annual follow-ups to monitor outcomes and identify opportunities for program improvement.

Qualifications
Education
  • Bachelor's degree required; Health Education preferred.
  • Master of Social Work (MSW) or Certified Health Coach credential preferred.

Experience
  • Minimum of 2 years of experience in community-based social work, case management, human services, employee assistance, community outreach, or resource coordination.
  • Experience conducting needs assessments, connecting individuals to resources, providing advocacy, and building community partnerships.
  • Prior experience with developing, implementing, and managing programs that promote health, well-being, and healthy lifestyle behaviors for a specific population, such as employees, health plan members, patients, students, or community residents.

Skills
  • Strong verbal and written communication skills.
  • Ability to support various shifts, including occasional evenings and weekends.
  • Strong problem-solving and organizational skills.
  • Ability to work effectively under deadlines and manage multiple priorities.
