Job description

Description
Responsible for coordination of healthcare training and carries a caseload. Ensures individuals medical needs are met in accordance with OPWDD regulations including medication regimes, medical/psychiatric conditions and routine follow-up medical care including optical and dental care.
POSITION RESPONSIBILITIES
  • Displays leadership and acts as a mentor within the department. Serves as a role model in adhering to Management Standards and Expectations. Displays ethical decision making.
  • Models and promotes Person Centered Planning, considering the dreams and goals of individuals being served.
  • Assists in training and mentoring Registered Nurses.
  • Coordinates and oversees the training of AMAP staff to include the Medication Administration Curriculum and Medication Pours.
  • Develops medical/healthcare training tools to include guidance materials for nutrition and personal healthcare for Direct Support Professionals. Provides recommendations, materials and input to the training department for staff training development.
  • Coordinates and trains staff on audit standards and agency policies related to medical care and treatment.
  • Oversees the medication error process by recommending and providing needed action for staff error including retraining of staff and support and direction to site managers in their supervision of staff. Identifies trends and incorporates information in trainings to enhance quality of service provision.
  • Provides site-specific training with regard to health and nutrition, medication, drug interaction/reactions and any other medical topics deemed appropriate.
  • Monitors each individual's healthcare clinical status and treatment response.
  • Evaluates each individual living in certified residential setting for ability to self-administer medications.
  • Works with multidisciplinary team in establishing goals and evaluation outcomes for each individual with regard to independence in medication administration.
  • Oversees site specific medication reviews weekly at residential sites to ensure the proper administration of medication by medication certified staff, targeting accuracy of administration and compliance with physician orders.
  • Identifies and shares best practices in the provision of quality health care.
  • Responsible for rotational on-call responsibilities for medical needs including consultation in medical emergency situations.
  • Attends site-specific staff meetings to ensure comprehensive treatment to all individuals as needed.
  • Acts as a primary member of the multidisciplinary treatment team to evaluate individual goals, outcomes and to facilitate person-centered planning.
  • Participates in Nursing Partnership Activities.

Requirements
Education and Experience
  • Licensed and registered by New York State Department of Education in Nursing.
  • Experience in OPWDD certified residential/day habilitation setting preferred.

Skills, Knowledge and Abilities
  • Knowledge of nursing, medical supplies, and medical terminology.
  • Good written and oral communication skills.
  • Excellent training, facilitation, mentoring and interpersonal skills.
  • Working knowledge of industry regulations and procedures.
  • Must be timely and prompt in performing tasks, as well as demonstrate the ability to exercise independent judgment and to meet deadlines.
  • Knowledge of person centered planning and approaches.
  • Basic computer knowledge and database management skills.
  • The ability to maintain orderly files and records.

Certifications
  • Must hold and maintain NYS Licensing requirements throughout employment and provide updates as required.

Verifications and Screenings
  • OPWDD clearance for convictions of criminal offenses.
  • A valid NYS Driver's License.
  • A drug screen prior to the commencement of employment.
  • Tuberculosis screening started or completed prior to the commencement of employment.
  • Approval of NYS Justice Center.

Physical and Mental Requirements
  • Seeing, hearing, walking, sitting, standing, stooping, stretching and driving.
  • Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines.
  • Staying awake and alert while on duty.

Working Conditions
  • Full time schedule to be coordinated with supervisor, expected 40 hours per week.
  • Part time schedule to be coordinated with supervisor.
  • Relief schedule to be coordinated with supervisor; Relief positions must work a minimum of 12 hours per month. Training hours do not count towards required hours.
  • Will usually work normal business hours. May be required to address emergency situations during evening or weekend hours.
  • Frequent local travel required. Mileage reimbursement provided.
