Home Helpers Home Care is looking for an exceptional LPN Home Care Coordinator to join our clinical and client care team. This part-time role combines nursing knowledge with client support, caregiver orientation and training, field visits, care coordination and quality assurance.
It's a great opportunity for an LPN who enjoys people, problem-solving and teaching-and wants the opportunity to grow into a larger role as our company continues to expand.

What You'll Do
You'll work closely with our clinical and client care teams to help ensure our clients receive exceptional, consistent care.
  • Conduct client home visits, wellness visits and follow-up
  • Assist with new client starts and changes in care
  • Monitor client conditions and communicate concerns to our clinical team
  • Assist with new caregiver orientations, training and skills assessments
  • Provide caregiver education, coaching and clinical support
  • Help ensure caregivers understand and follow client care plans
  • Support caregiver competency and skills development
  • Communicate with clients, families, caregivers and our internal team
  • Review documentation and support quality assurance and regulatory compliance
  • Provide hands-on clinical support within LPN scope of practice when appropriate

Who We're Looking For
We're looking for more than clinical experience. We're looking for someone who is warm, capable, dependable and great with people
  • Current Georgia LPN license
  • Home care, home health, senior care, assisted living, skilled nursing or related experience preferred
  • Strong clinical judgment and communication skills
  • Comfortable teaching, coaching and assessing caregiver skills
  • Organized, dependable and detail-oriented
  • Comfortable working independently in the field and collaboratively with a team
  • Takes initiative and follows through
  • Interested in learning, growing and taking on additional responsibility
    Home care experience is preferred, but we'll consider an exceptional LPN with transferable experience and the right approach to people.
