We are seeking a compassionate and skilled Psychiatric Nurse Practitioner to join our interdisciplinary team. The ideal candidate will provide high-quality psychiatric evaluation, diagnosis, and management of mental health conditions for residents in skilled nursing facilities, collaborating closely with physicians, therapists, and other healthcare professionals to enhance patient well-being and quality of life.
Key Responsibilities
  • Conduct comprehensive psychiatric assessments and evaluations of residents, diagnosing mental health disorders and developing individualized treatment plans.
  • Prescribe and manage psychotropic medications, monitoring effectiveness and side effects while adjusting therapies as needed.
  • Collaborate with interdisciplinary teams including physicians, psychologists, social workers, and nursing staff to coordinate holistic patient care.
  • Provide psychotherapy and counseling services where applicable to support residents' mental health and coping strategies.
  • Participate in care plan meetings, quality improvement initiatives, and value-based care programs aimed at enhancing psychiatric outcomes and reducing hospital readmissions.
  • Educate residents, families, and facility staff on mental health conditions, treatment plans, and behavioral interventions.
  • Maintain accurate and timely documentation in compliance with clinical and regulatory standards.

Requirements
  • Master's or Doctorate degree in Nursing with specialization in Psychiatric-Mental Health Nurse Practitioner (PMHNP) from an accredited program.
  • Active and unrestricted Advanced Practice Nurse (APN) license and certification as a Psychiatric Nurse Practitioner.
  • Current DEA registration with prescriptive authority for controlled substances.
  • Minimum of 2 years clinical experience preferred, especially in geriatrics, long-term care, or psychiatric settings.
  • Strong knowledge of psychiatric disorders, psychopharmacology, and evidence-based mental health practices.
  • Ability to work collaboratively within a multidisciplinary team and independently manage patient caseloads.
  • Excellent communication and interpersonal skills with patients, families, and healthcare team members.
  • Familiarity with electronic health records (EHR) systems and commitment to accurate clinical documentation.
