Job description

We are seeking full time and part-time RN LPN PT OT to join our team! You will be responsible for delivering high quality care to assigned patients.
Rehab At Home is a fast growing home health agency that services Marion, Lake, and Sumter counties. We are currently looking for experienced nurses to join our team. We are willing to train nurses who are willing to learn and grow with us. Our clients include those in need of nursing and therapy care post-hospitalizations, post-surgery, chronic disease exacerbation, wound care, ostomy care, Foley catheter management, IV therapy. Visit rate and mileage is competitive, and we are willing to offer you individualized incentive pay unique to your skills and experience. Our agency is case-managed by a Nurse Practitioner as the Director of Clinical Services, a unique advantage which sets us apart. .You will be working with a well-experienced team who believes that patient care comes first, in order to achieve exceptional outcomes. We have full time and part time opportunities available. Additional on-call assignments also available on weekends in addition to per visit rate and mileage.

  • Care for ill, injured, or convalescing patients
  • Provide basic patient care and treatment as part of disease management
  • Medication management and reconciliation
