Title: Home Health Pediatrics RN or LPN
Job Title: Licensed Practical Nurse or Registered Nurse

Rate: $30-$35 (LPN) | $45-$50 (RN), as W-2 employee
Reports To: Clinical Supervisor
Location: Greater Northern Atlanta area
Position OverviewWe are seeking Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to support the growth of our home health agency.
In this role, you will provide one-on-one nursing care for children with complex medical needs in their homes. Common conditions include ventilator dependence, G-tube feedings, and seizure disorders. Responsibilities include administering medications, monitoring vital signs, performing tracheostomy and wound care, and educating families on home care procedures. You will work under the supervision of a Clinical Supervisor, develop strong relationships with families, and maintain accurate, detailed patient records.
Key Responsibilities
  • Provide professional nursing care for medically fragile children ages 0-21 in a home setting, often on long-term assignments.
  • Maintain physician orders for clients and document all changes in the medical record per Agency procedures.
  • Deliver comprehensive clinical care, including personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric care (tracheostomy, ventilator management, and tube feedings).
  • Collaborate effectively with other members of the client's care team.
  • Promote safety and maintain a safe environment for clients and coworkers.
  • Perform duties in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
  • Participate in care coordination and attend agency meetings as requested.
  • Perform other related duties as assigned.

Qualifications
  • Education: Graduate of an accredited nursing program (BSN preferred).
  • Licensure: Current Georgia LPN or RN license
  • Experience:
    • Strong knowledge of health regulations and documentation requirements.
  • Skills:
    • Excellent communication, organizational, and interpersonal skills.
    • Proficiency with electronic EMR systems.
  • Certifications: Current CPR certification.
  • Other Requirements: Licensed driver with insured vehicle (as required by Georgia law).
