Oasis Therapy Services is seeking an Occupational Therapist (OT) to provide one-on-one rehabilitation services in Apple Valley, Minnesota.

Responsibilities

  • Evaluate residents and develop individualized treatment plans.
  • Provide therapy focused on ADLs, mobility, safety, and independence.
  • Monitor progress and adjust treatment plans as needed.
  • Educate residents and caregivers on exercises, adaptive techniques, and equipment.
  • Travel to Cannon Falls or provide telehealth support there as needed.
  • Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

  • Active and current Minnesota Occupational Therapist license.
  • Graduate of an accredited Occupational Therapy program.
  • Current CPR certification.
  • Strong communication, organization, and documentation skills.
  • Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
