DISCLOSURE - MULTI-EMPLOYER ROLE
This W-2 hourly Family Nurse Practitioner (FNP) position includes responsibilities that span across the following affiliated organizations, all under shared leadership:
  • Community Wellness Outpatient Mental Health Center
  • Community Wellness Psychiatric Rehabilitation Program
  • Lighthouse Behavioral Health Center LLC
  • Lighthouse Behavioral Health Center at 1116 E 20th Street LLC
  • Lighthouse Behavioral Health Center at 1723 Gwynns Falls Parkway LLC
  • Freedom Healthcare LLC

You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of multiple affiliated companies, with hours, responsibilities, and compliance obligations tracked separately for each.
This role is structured as a 16-hour per week, part-time position. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care, each employer will manage its own onboarding, HR, and compliance processes in accordance with applicable laws and organizational policies.

CLASSIFICATION: Part-time, W-2 employee (16 hours per week across scheduled days)

  • Medication Assisted Treatment Nurse Practitioner (6.25% / 1 hr) - Lighthouse Behavioral Health Center, LLC
  • Somatic Nurse Practitioner (6.25% / 1 hr) - Community Wellness Outpatient Mental Health Center
  • Nurse Practitioner Consultant (30% / 4.8 hrs) - Community Wellness Psychiatric Rehabilitation Program
  • Clinic Director / Family Nurse Practitioner / Operations Manager 12 (32.5% / 5.2 hrs) - Freedom Healthcare, LLC
  • Nurse Practitioner (12.5% / 2 hrs) - Lighthouse Behavioral Health Center at 1116 E 20th Street LLC

WORK SCHEDULE
  • Tuesday and Thursday 8:00 AM - 5:00 PM (16 hours per week)
    On-call availability may be required for after-hours emergencies.

COMPENSATION
Hourly pay ranges from $30.00 to $90.00, commensurate with experience, expertise, verified credentials, and available company budget.

ANTICIPATED TRAVEL
Minimal (approximately 5% of the time). Travel may be required between program sites in Maryland. No additional compensation is provided for travel.
PHYSICAL DEMANDS

SUMMARY OF POSITION RESPONSIBILITIES
The Family Nurse Practitioner provides comprehensive primary and integrated healthcare services to individuals enrolled in outpatient behavioral health, psychiatric rehabilitation, substance use treatment, and primary care programs. This role is responsible for performing medical evaluations, managing acute and chronic health conditions, prescribing medications (including MAT when indicated), and coordinating care with interdisciplinary teams.
The FNP ensures that all services meet COMAR regulations, OHCQ standards, and Maryland Health-General Article Title 19 requirements while supporting integration of behavioral and somatic health services. The FNP is authorized to bill CPT codes 99202-99205 and 99211-99215 for eligible services rendered.
This position plays a critical role in bridging the gap between primary care, mental health, and substance use treatment, ensuring that clients receive safe, person-centered, and regulatory-compliant services.
SCHEDULED DUTIES AND RESPONSIBILITIES
  • Conduct face-to-face medical evaluations, wellness visits, and follow-up care for enrolled clients.
  • Diagnose and manage acute and chronic health conditions; prescribe medications within scope of practice.
  • Perform initial and ongoing MAT evaluations, including induction, stabilization, and monitoring, as needed.
  • Order, review, and interpret diagnostic tests, labs, and urine drug screens.
  • Review, document, and update comprehensive medical histories, including conditions, allergies, communicable diseases, and medications.
  • Develop individualized treatment and follow-up plans, including referrals to primary care providers or specialists.
  • Educate clients and families on disease prevention, wellness strategies, relapse prevention, and safe medication use.
  • Participate in interdisciplinary team meetings, case reviews, and staff trainings.
  • Document all encounters in the EHR (ICANotes) in compliance with COMAR, Medicaid, and payer standards.
  • Support audit readiness, licensing inspections, and quality assurance initiatives.

UNSCHEDULED DUTIES AND RESPONSIBILITIES
  • Respond to after-hours emergencies when delegated or on-call.
  • Act as clinical consultant for complex cases requiring coordination with outside providers.
  • Participate in peer reviews, documentation audits, and policy updates.
  • Provide staff coaching and consultation on medication protocols, infection control, and resident safety.
  • Represent the organization in family meetings, licensing reviews, and community partner engagements.
  • Carry out additional duties as assigned by the Medical Director or Operations Leadership.

COMPETENCIES AND SKILLS
  • Strong clinical skills in primary care, integrated care, and addiction medicine.
  • Knowledge of COMAR, OHCQ, CARF, SAMHSA, and DEA regulations.
  • Proficiency in EHR systems (ICANotes preferred) and CPT coding.

LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS
  • Active Maryland Board of Nursing (MBON) licensure as a Certified Registered Nurse Practitioner (CRNP).
  • Master's degree in Nursing from an accredited institution.
  • Board certification as a Family Nurse Practitioner (FNP).
  • Valid DEA registration and CDS (Controlled Dangerous Substances) registration.
  • Minimum 2-3 years of clinical experience in primary care, behavioral health, or integrated healthcare settings.
  • Experience with MAT services strongly preferred.
  • Active CPR/First Aid certification.
  • Ability to maintain professional liability/malpractice insurance.
