UPLIFT HOMECARE, LLC is seeking compassionate, skilled, and dependable Registered Nurses (RN) and Licensed Practical Nurses (LPN) to join our growing home health team serving Polk County and surrounding areas.

Responsibilities

As a Home Health RN or LPN, you may:

  • Provide skilled nursing services in accordance with physician orders and the patient's individualized plan of care.
  • Participate in patient assessments and ongoing evaluation of patient needs.
  • Monitor vital signs and patient condition.
  • Administer medications, treatments, injections, and other skilled nursing services within your scope of practice.
  • Observe and report changes in patient condition to the appropriate physician, Clinical Manager, Case Manager, or supervising nurse.
  • Provide patient and caregiver education regarding medications, disease management, treatments, safety, and health maintenance.
  • Assist patients with maintaining independence and managing their healthcare needs at home.
  • Complete accurate, timely, and compliant clinical documentation using the agency's EMR/EHR system.
  • Communicate effectively with physicians, therapists, case managers, and other members of the interdisciplinary care team.
  • Maintain compliance with HIPAA, agency policies, and applicable state and federal regulations.
  • Participate in care coordination and interdisciplinary communication.
  • Participate in quality improvement activities, in-services, and required training.
  • Promote patient safety, dignity, independence, and quality of life.

RN-Specific Responsibilities

Registered Nurses may additionally be responsible for:

  • Completing comprehensive nursing assessments.
  • Developing and evaluating individualized plans of care.
  • Performing OASIS assessments as required.
  • Coordinating patient care with physicians and the interdisciplinary team.
  • Providing clinical oversight and guidance as appropriate.
  • Using clinical judgment to identify changes in patient condition and determine appropriate interventions.

LPN-Specific Responsibilities

Licensed Practical Nurses will provide nursing services under the direction and supervision required by applicable regulations and the patient's plan of care, including:

  • Implementing nursing interventions established in the plan of care.
  • Monitoring and reporting patient status and changes in condition.
  • Administering medications and treatments within the LPN scope of practice.
  • Reinforcing patient and caregiver education.
  • Maintaining accurate clinical documentation.
  • Communicating effectively with the supervising RN, physician, and care team.

Qualifications

Registered Nurse (RN)

  • Current, unrestricted Florida RN license.
  • Minimum 1 year of home health experience preferred/required based on assignment.
  • OASIS/home health assessment experience preferred.
  • Current CPR certification.
  • AHCA Level II Background Screening.
  • Valid driver's license.
  • Reliable transportation.
  • Current auto insurance.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and documentation skills.
  • Ability to work independently in a home health environment.

Licensed Practical Nurse (LPN)

  • Current, unrestricted Florida LPN license.
  • Home health experience preferred.
