Job Summary
We are seeking a Registered Nurse to join our Home Health team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.  If you take pride in providing high-quality, one-on-one clinical care and have a heart for serving your community, we want to meet you.

Responsibilities 

  • Patient Assessment: Conduct comprehensive initial and ongoing assessments of patient status.
  • Care Coordination: Collaborate with ordering physicians, therapists, and family members to develop and implement effective care plans.
  • Clinical Care: Administer medications, perform wound care, and monitor vital signs, documenting all changes in condition promptly.
  • Education: Empower patients and caregivers by providing education on disease management, medication adherence, and safety protocols.
  • Documentation: Maintain accurate, up-to-date electronic medical records (EMR) in compliance with state and federal regulations.
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 

  • Administer medications and monitor patients for side effects or reactions  

  • Document any changes in patient conditions and report on changes

  • Maintain accurate records of patient care and discuss observations with the supervisor  
Qualifications/Requirements
  • Currently licensed Registered Nurse (RN) in state of employment 
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver’s license 
  • First aid/CPR certification required 
  • Traveling to patient home around the Houston area
