Job description

Hourly Rates Range: $35.00/hr - $45.00/hr. based on experience/discipline. 

Opportunity: 

  1. Pediatric Client - 4 year old female in 28202. Every Tuesday & Thursday 7:30am-4:30pm. Has down syndrome with g-tube, med administration and assistance with ADLs. HAS CHALLENGING BEHAVIORS 
  2. Veteran client - 71 year old male client in 28227. Every Saturday 8:30am-4:30pm. Requires help with shower, apply prosthetic, meal prep, assistance with ambulation, monitor skin integrity, check blood sugar, administer insulin, administer medication/breathing treatments, head to toe assessment. 
  3. Assessments/Reassessments - Looking for a RN who can help open up new cases and reassess existing clients. 
  4. Infusion/Wound care - RN who is comfortable doing both. 

Schedule: Part Time/Full Time opportunities available

Responsibilities

  • Provide thoughtful care to clients & patients based on personalized Assessments and Plan of Cares
  • Monitor and track changes in the client/patient’s conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
  • Proactively updates client/patients’ family on plan of care, health changes, and concerns

Requirements:

  • Current unrestricted RN license in state for which care is being provided
  • Certified CPR and negative TB skin test or chest x-ray
  • Minimum one (1) year of documented experience required in a home health agency, infusion experience (required), primary care clinic or health facility, preferred.
  • Possess exceptional nursing assessment skills
  • Analytic/Critical thinker.
  • Licensed driver with valid automobile insurance
  • Reliable transportation
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds.
