Job Summary
We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in performing personalized care for patients and are able to observe and communicate effectively in your patient's best interest. Must be willing to travel. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for learning new skills. 
Responsibilities 
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 
  • Administer wound care and monitor patients for improvement or decline
  • Document any changes in patient conditions and report on changes 
  • Maintain accurate records of patient care and discuss observations with the Providers  
  • Utilize electronic health record (EHR) systems like WoundAlly  for accurate documentation of patient care activities and medical records.
  • Visiting patients home, facilities, etc. in the Los Angeles County, Orange County, Inland Empire and surrounding areas.
Qualifications
  • Currently licensed Registered Nurse (RN) in state of employment 
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver’s license 
  • First aid/CPR certification required
  • Wound Care certificate (preferred)
  • Proof of personal liability
  • Car Insurance
