Resident Services Coordinator (Part-Time)

Everyone deserves a home where they are valued, cared for, and supported to live their most independent and fulfilling lives. All Macdonald Residence residents have complex needs and often a history of behavioral health and addiction challenges. As the Resident Services Coordinator, you will not only be working as a part of a cohesive and interdisciplinary resident care team, but you will also be supporting the facility in its ongoing endeavor to deliver comprehensive, holistic health models. You will be supported by caring and competent individuals who are passionate about the Macdonald Residence community.

The Resident Services Coordinator assists residents with daily tasks, ranging from finances to medical appointments. Additionally, this position entails providing a great deal of daily social support to residents while building and maintaining relationships with residents to foster independence and enhance the quality of residents' lives.

Under the guidance of the Resident Services Manager, the Resident Services Coordinator carries out communication with community agencies, caseworkers, and family members on behalf of the residents. Additionally, the Resident Services Coordinator treats each resident with respect and dignity, recognizing individual needs and encouraging independence, as well as fostering a homelike atmosphere throughout the facility.

Duties and Responsibilities

The individual in this position is expected to engage in the following work-related activities.

  • Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for residents.
  • Escorts residents to and from appointments.
  • Attends resident appointments and provides support.
  • Help residents navigate complex systems and paperwork (i.e., Social Security, Vital Records, Cell phone service)
  • Engages residents in meaningful activities and discussions.
  • Prioritizes resident needs and addresses those needs in a timely manner.
  • Assists residents with finances and rent payments.
  • Works cohesively with Life Enrichment staff and the Resident Services staff to deliver a holistic model of psychosocial care to residents
  • Assists with Life Enrichment activities.
  • Document pertinent information in residents' charts per policy (i.e., progress notes and incident reports).
  • Contributes pertinent information to resident service plans and activity plans.
  • Adheres to emergency protocols, including performing CPR and administering first aid as necessary.
  • Attend education and training classes to provide excellent care and meet state and other requirements.
  • Completes online training assignments as required in a timely manner.
  • Reads staff communication records and maintains familiarity with resident records as required.
  • Reviews and signs off on all Service Plans quarterly and Behavior Plans and Temporary Service Plans daily.
  • Provides services as indicated on Service Plans, Activity Plans, and Behavior Support Plans.
  • Observes problems, concerns, or issues with resident apartments, the facility, and the grounds and communicates them appropriately.
  • Observes changes in residents' statuses, needs, or preferences, promptly reporting all changes to the appropriate party.
  • Possesses thorough knowledge and adheres to all fire, life safety, disaster, security, work safety, and emergency procedures, prioritizing resident safety in critical situations.
  • Adheres to all established policies and procedures.
  • Avoids loss, breakage, and waste of supplies and equipment.
  • Assures effective communication between Direct Care staff and others in the facility - nurses, behavioral health staff, administrator, etc.
  • Supports a dignified and caring atmosphere with residents, residents' families, visitors, and staff.
  • Protects the privacy and confidentiality of information pertaining to the resident, employee, residence, company information, and records.
  • Contributes to a strong, cohesive team environment.
  • Maintains a safe and secure working environment and practices safe working habits.
  • Participates in trainings, in-services, and attends meetings as required.
  • Assists with projects as assigned and performs other related duties.
Job Qualifications

The following qualifications are required to perform the essential functions of this position.

  • Must have a minimum of 6 months related experience and/or training; prefer 2 years of experience working with residents in a community-based care setting.
  • Experience working with residents with behavioral health challenges, substance use, and histories of houselessness.
  • Valid driver's license and clean driving record to transport residents for appointments, outings, and activities.
  • Ability and willingness to drive a large van.
  • Ability and willingness to perform all position responsibilities adequately.
  • Advanced knowledge of Microsoft Office to utilize technology effectively for documentation, resident engagement, and other tasks.
  • Openness to learn new systems, equipment, and best practices to continuously improve resident care.
  • Maintains acceptable attendance record and follows work schedule.
  • First Aid Certification, CPR Certification, dementia certification preferred, but training will be provided during the new employee onboarding period.
