Jewish Family & Career Services of Atlanta (JF&CS) is seeking a skilled, dependable Licensed Practical Nurse (LPN) to support individuals with intellectual and developmental disabilities by providing health-related oversight, medication coordination, and nursing support in community-based programs.

With a 125-year history of serving individuals and families of all faiths, JF&CS is a trusted community resource. In this role, you’ll help ensure quality care, safe routines, regulatory compliance, and clear coordination between individuals, staff, families, and medical providers.


The north star for this role is simple: keep individuals safe, stable, and supported through consistent, high-quality nursing oversight.


The Role at a Glance

  • Part-time position
  • Community-based work across program sites; travel and home visits required
  • Mix of independent work and collaboration with program staff and leadership
  • Hands-on clinical coordination, documentation, and staff support
  • Reports to: Registered Nurse (Developmental Disabilities Division Nurse)

What You’ll Be Responsible For


Healthcare Oversight & Compliance

  • Provide healthcare oversight and documentation to maintain compliance with agency policies and procedures, Medicaid waiver services, CARF standards, and applicable regulations for program participants.
  • Ensure health care protocols are current and followed.

Medication & Care Coordination

  • Coordinate medication management for individuals in the independent living program.
  • Consult with staff, individuals, families, and medical professionals to facilitate health care management.
  • Coordinate appointments and monitor annual physicals.

Assessments, Visits & Follow-Up

  • Conduct and document supervisory home visits at least every 60 days (and as needed).
  • Provide follow-up supervision with individuals and staff to resolve health-related concerns.
  • Promptly report health-related issues identified during assessments or relayed by staff to the RN.

Training & Team Support

  • Conduct and document annual skill checks for staff as needed.
  • Provide in-service trainings for staff as needed.
  • Attend Individual Service Plan (ISP) meetings as needed.

Documentation & Scheduling

  • Maintain client care plans, physicals, and the supervisory home-visit schedule.
  • Maintain accurate documentation to support quality care and compliance.

On-Call Coverage

  • Manage the on-call nursing emergency cell phone in rotation with the department RN, including overnight, weekend, and vacation coverage.
Requirements:

You have:

  • Active, in-good-standing Georgia LPN license (required)
  • Strong interpersonal and communication skills

  • Ability to work independently and as part of a team
  • Active, in-good-standing Georgia driver’s license (required)
  • Ability to maintain required 16 h annual training (DBHDD)

Education & Experience

  • 5 years of experience as a Licensed Practical Nurse (required)
  • Experience working with individuals with developmental disabilities (preferred)
  • Computer proficiency (required)

Physical Demands

High energy level; ability to work with a diverse population with tact, maturity, and flexibility. Ability to manage multiple tasks and assignments; ability to concentrate and focus on information, reports, and outcome measures; and comfort working in a high-performance team environment. Physical requirements include sight, hearing, sitting for more than four hours each day, lifting 50 lb or less (per HRST requirement) and other physical requirements required to complete essential functions of the position. Must be able to work a flexible schedule.

May vary from site to site as the employee travels frequently for home visits, meetings, and events.
