Job description

Nephrologist (Remote) - Houston, TX (Offering Relocation & Sign-on Bonus)
This position is primarily remote, with occasional in-person visits to patient homes. The position is full-time, Monday through Friday, with no options for part-time, locum, weekend, or contract work.

Schedule: Full-time position, Monday-Friday 8:30 am-5:00 pm
Pay Range: $250k - $300k

Job summary: We are seeking a full-time Lead Nephrologist to join their team in delivering value-based care to patients with chronic kidney disease, end stage renal disease, and/or other polychronic conditions. The nephrologist will see patients in person in patient homes and via telehealth.
Requirements  :
·         Medical degree from an accredited medical school. 
·         Board certification in Nephrology. 
·         Current state medical license and DEA registration without restrictions
·         Strong clinical skills and experience in caring for patients with kidney diseases across a variety of settings (inpatient, outpatient, dialysis, etc.)
·         Commitment to providing patient-centered care and promoting cooperative decision-making. 
·         Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare teams. 
·         Experience with value-based care models, quality improvement methodologies, and population health management initiatives preferred.
