Job Title
Licensed Master Social Worker (LMSW) – Part-Time / PRN
Job Summary
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our Home Health and Hospice team on a Part-Time or PRN basis. The LMSW will provide psychosocial assessments, counseling, emotional support, resource coordination, and advocacy services to patients and families experiencing illness, disability, end-of-life care, and bereavement. The LMSW will work collaboratively with physicians, nurses, therapists, and other interdisciplinary team members to ensure high-quality patient-centered care.
Essential Duties and Responsibilities
  •  Conduct comprehensive psychosocial assessments of patients and families. 
  •  Evaluate social, emotional, financial, environmental, and community support needs. 
  •  Develop and implement individualized care plans in collaboration with the interdisciplinary team. 
  •  Provide counseling, crisis intervention, and emotional support to patients and caregivers. 
  •  Assist patients and families in accessing community resources, financial assistance programs, and support services. 
  •  Educate patients and families regarding available healthcare, hospice, and community services. 
  •  Facilitate advance care planning and discussions regarding patient goals and preferences. 
  •  Participate in interdisciplinary team meetings and contribute to patient care planning. 
  •  Maintain accurate, timely, and compliant clinical documentation. 
  •  Coordinate discharge planning and transitions of care when appropriate. 
  •  Provide bereavement support and grief counseling to families and caregivers. 
  •  Ensure compliance with federal, state, agency, Medicare, and hospice regulations. 
  •  Maintain patient confidentiality in accordance with HIPAA requirements. 
  •  Perform other duties as assigned by the Clinical Director or Administrator. 
Qualifications
Education
  •  Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE). 
Licensure
  •  Current Licensed Master Social Worker (LMSW) license in the State of Texas. 
  •  Current driver's license and reliable transportation. 
  •  Current automobile liability insurance. 
Experience
  •  Minimum one (1) year of social work experience preferred. 
  •  Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred. 
  •  Knowledge of Medicare, Medicaid, and community resources preferred. 
Skills and Abilities
  •  Strong assessment, counseling, and crisis intervention skills. 
  •  Excellent verbal and written communication abilities. 
  •  Ability to work independently and as part of an interdisciplinary team. 
  •  Strong organizational and time-management skills. 
  •  Compassionate and empathetic approach to patient care. 
  •  Proficiency with electronic medical records (EMR) systems.

Physical Requirements
  •  Ability to travel to patient homes, facilities, and community locations.
