Responsibilities:
  • Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.
  • Conduct comprehensive assessments of patients' social and emotional needs
  • Assist with coordination of care and access to community resources
  • Maintain detailed and accurate documentation of patient interactions and progress
  • Utilize Homecare Homebase EMR (knowledge of which is a plus)
  • Develop personalized care plans in collaboration with the interdisciplinary team
  • Provide counseling and support to patients and families facing end-of-life challenges

Requirements
  • Licensed as an LMSW
  • Knowledge or experience utilizing Homecare Homebase EMR would be a plus
  • Minimum of one year of hospice care experience
  • Minimum of one year of social work experience
  • Compassionate and empathetic approach to working with the elderly
  • Strong clinical and assessment skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and manage time effectively

Payrate: $55-$75 per visit
