Summary
Star Valley Health is seeking a compassionate and skilled Registered Nurse to join our team in Bridger Valley. This unique role combines direct patient care with care coordination responsibilities, offering the opportunity to make a meaningful impact on patients' lives across the continuum of care. You will partner with patients, families, providers, and interdisciplinary teams to deliver high-quality, patient-centered care, while helping manage chronic conditions, facilitate smooth care transitions, and promote preventative health strategies.

Key Responsibilities
  • Deliver exceptional nursing care using the nursing process: assessment, diagnosis, planning, implementation, and evaluation.
  • Administer medications and treatments per physician orders and hospital policy; interpret diagnostic tests and adjust care plans as needed.
  • Provide pain assessment and management, educating patients and families on treatment options.
  • Coordinate care for patients with chronic conditions, ensuring early detection and effective management.
  • Stratify patient populations by risk and implement care plans to reduce avoidable acute illness.
  • Facilitate Annual Wellness Visits (AWV), Chronic Care Management (CCM) encounters, and care transitions.
  • Conduct daily huddles with providers and care teams to identify patient needs and scheduling opportunities.
  • Deliver clinical health coaching to support patient self-management and lifestyle changes.
  • Mobilize community resources and establish counseling services for obesity, tobacco cessation, diabetes, cardiovascular health, and behavioral health.
  • Maintain accurate documentation for compliance and billing purposes.
  • Support a culture of safety, quality, and patient-centered care.

Requirements
Qualifications
  • Education & Licensure:
    • Graduate of an accredited nursing program.
    • Current Wyoming RN license required.
  • Certifications:
    • AHA Basic Life Support (BLS) required.
    • PALS and NRP (or ability to obtain within 6 months).
  • Experience:
    • Previous experience in chronic disease management and direct patient care preferred.
    • Background in Care Coordination, Case Management, or Behavioral Health a plus.
  • Skills:
    • Strong clinical assessment and critical thinking skills.
    • Excellent communication, organization, and leadership abilities.
    • Ability to manage high-volume caseloads and adapt to changing priorities.
    • Familiarity with health IT systems and data reporting preferred.
