Job description

If you are a hospice compliance social work professional with a background in survey readiness and passion for improving the quality of patient care, this is a great opportunity for you to make an impact on a national scale!

At Community Health Accreditation Partner (CHAP), you’re not just taking on a role, you’re becoming part of a mission-driven organization committed to excellence, integrity, and flexibility. We cultivate a supportive, collaborative culture where your expertise is valued, and your work truly makes a difference.

This is a nationwide, travel-based, per-diem position. All assignments will require travel to a hospice agency being surveyed for accreditation/certification.

CHAP Site Visitors (surveyors) are responsible for conducting, as well as the planning, organization, and coordination of community-based healthcare accreditation and/or certification surveys. These visits must be performed in accordance with the CHAP Standards of Excellence. The visits must be executed in compliance with CHAP policies and procedures that govern the accreditation/certification process and must ensure confidentiality except where required by law. Hospice IPU/HCFC/LSC site visitors are required to complete required CHAP internal and QSEP LSC orientation and applicable training in accordance with CHAP Site Visitor Education and Evaluation Policy prior to conducting independent Hospice IPU/LSC site visits.
Successful candidates must possess strong working level knowledge of the CMS Hospice Conditions of Participation (full scope of the CoPs) and resources related to the CoPs. A knowledge of the CHAP Standards of Excellence is a plus. This position requires strong attention to detail and ability to deliver accurate, complete, and well-written documentation. Candidates must be able to work autonomously, apply critical thinking, and be adaptable to change.
Minimum Qualifications:
  • Bachelor's degree in related field required; Master's degree preferred.
  • Current Social Work license required.
  • Minimum of 5 years of compliance experience within the hospice industry.
  • At least 1 year of leadership experience, including responsibility for leading and/or supervising others.
  • Candidates must have prior experience working in a healthcare facility, such as:
    • Hospice inpatient units (IPUs)
    • Skilled nursing facilities (SNFs)
    • Healthcare facility administration
    • Life Safety Code (LSC) compliance
    • Other healthcare regulatory or compliance-related roles

Availability Requirements:

  • Must be available for a minimum of 5 survey days per month (travel days do not count toward survey days).

Benefits:

  • CHAP allows all employees to participate in our 401(k) plan, our free identity theft protection, and other wellness benefits at little to no cost.

Per Diem Pay structure is as follows:

Survey days: $402/day

Travel days: $165/day (1 day per assigned survey)

Site Visit prep time: $75/per site visit

Other: $60/per day in meal per diem per site visit day, $30 per day in meal per diem for each travel day.

Example of minimum weekly pay: $1656.00 including meal stipends *(based on a three-day workweek and one travel)

Travel: All airfare expenses are company paid; other travel expenses are reimbursed including mileage at government rate. Employee earns all travel credits (airline, hotel, rental car points).
