Registered Nurse (RN) - Hospice

Pay: $60.00 - $67.00 per hour

Schedule: Part time, weekends

Coverage Area: Alameda County, Santa Clara County, Peninsula

We're growing and looking for an experienced Registered Nurse (RN) to join our Hospice Care team!

What You'll Do

As a Hospice RN, you will plan, organize, and direct hospice care while educating patients and families. You'll serve as a case manager and ensure quality, compassionate care in the home setting.

Key Responsibilities:

  • Complete initial and ongoing patient and family assessments

  • Develop and revise individualized hospice care plans

  • Provide professional nursing care using the nursing process

  • Administer medications and treatments as prescribed

  • Educate and counsel patients and families on hospice care

  • Coordinate care for your assigned caseload

What We're Looking For

  • Active CA RN License

  • Minimum 2 years of nursing experience (hospice or home health experience preferred)

  • Strong clinical, communication, and problem-solving skills

  • Current CPR certification

  • Valid driver's license, insured vehicle in good condition

  • Ability to work independently and manage priorities
