Description
The is a PART TIME position. Vacancy is in the Health and Human Services Department to support the Chronic Disease Self-Management Program servicing adults age 65 and older. Under supervision, interviews applicants to obtain identifying information and to determine eligibility status in accordance with prescribed standards. Interviews applicants in their homes, community organizations, senior centers, offices, and other appropriate locations. Determines eligibility and connect older adults with appropriate health and community resources. Serve as an important link between seniors, health care providers, social service agencies, caregivers, and community organizations. Maintains statistical records on children/families for monthly, quarterly and annual reports.

This is a PART TIME 1716 position.
This is a grant-funded position, renewable on an annual basis. The City of Hartford is not obligated to fund this position upon termination of present funding.
Knowledge, Skills & Abilities
The examination will consist of a rating of your training and experience as contained on your application and may include a written test, an oral test, a performance test, or a combination thereof. All parts of the examination, including tests and ratings, will be related to the requirements of the position. The examination is designed to measure the following:
Knowledge of:
  • Basic principles of municipal welfare programs.
  • Rules, regulations, and laws affecting eligibility for general welfare and medical assistance.
  • Social and welfare agencies, institutions and clinics in the community.
  • Techniques of interviewing and methods of establishing client's financial status and eligibility.

Ability to:
  • Establish and maintain effective working relationships with clients, representatives of other agencies, other employees and the general public.
  • Express oneself clearly and concisely, orally and in writing.

Qualifications
Open to all applicants who meet the following qualifications:
Graduation from a standard high school, supplemented by college courses and one (1) year experience as an Interviewer, or three (3) years work experience involving public contact in the field of social services.
A COPY OF YOUR DIPLOMA OR TRANSCRIPT MUST BE ATTACHED TO YOUR APPLICATION. A DIPLOMA OR TRANSCRIPT SUBMITTED WILL BE VERIFIED BY THE HUMAN RESOURCES DEPARTMENT. APPLICATIONS WITHOUT PROPER DOCUMENTATION WILL NOT BE PROCESSED.
If selected, you will be required to pass a physical examination administered by a City physician, a drug and alcohol screening, and a background check. This examination and employment process is subject to all federal, state, and municipal laws, rules and regulations.
