Your Role:
  • Conduct in-home patient assessments and develop individualized care plans.
  • Administer medications, treatments, and wound care as prescribed.
  • Educate patients and families on managing health conditions and promoting independence.
  • Monitor patient progress and update care plans as needed.
  • Supervise LPNs and Home Health Aides to ensure quality care.
  • Participate in interdisciplinary meetings to coordinate comprehensive care.

What You Need:
  • Active Registered Nurse (RN) license in the State of Arizona.
  • Minimum of one year of nursing experience (home health, hospice, or critical care preferred).
  • CPR certification and valid driver's license with reliable transportation.
  • Excellent communication, organizational, and interpersonal skills.
