The Community Healthcaring KW provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion.

The Community Healthcaring KW works with the most complex client population served by primary health care professionals within Waterloo Region. Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community. We collaborate with many partner agencies and organizations within Waterloo Region. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:


Job Title: Registered Dietitian - 2 Days a week

Pay: $35.27 to $41.05 per hour

Part-Time, Fixed Term Contract

FTE 0.4 (15 hours a week) for 12 months


Job Summary

The Registered Dietitian (RD) works as a member of the interdisciplinary team that supports the provision of primary care for a wide range of complex clients. The Registered Dietitian provides community based nutritional and health assessments, nutritional care services, to support our complex populations.


Responsibilities

  1. Provide individual education on nutrition and health assessments.
  2. Develop client-focused nutritional plans.
  3. Conduct group education sessions focused on nutrition.
  4. Offer counseling and teaching tailored to clients' needs.
  5. Support behavior changes within the scope of practice defined by the College of Dietitians of Ontario.
  6. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  7. Support both primary care and refugee health-based patients.
  8. Use EMR and other relevant technology appropriately to support patients.
  9. Work collaboratively to perform community nutrition needs assessments and identify barriers to acquiring adequate food security and facilitate and support community initiatives to improve food
  10. Maintain accurate and up-to-date clinical notes in accordance with College guidelines and CHCKW
  11. Maintain current certification in Standard First Aid Level C, including CPR.
  12. Work during both regular and extended hours (evenings/weekends) and in locations within Kitchener to be determined by the Health Centre. An evening may be required.
  13. Maintain a valid driver's license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with CHCKW Policy.
  14. Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
  15. Collect, analyze and report on data and relevant information and participate in research as determined by
  16. Use interpreters/translators as needed and effectively.
  17. Collaborate with other team members to de-escalate situations or support clients while in crisis especially in the moments until added support is available.
  18. Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
  19. Work in a manner that preserves privacy and confidentiality.
  20. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
  21. Work in a manner consistent with Health Centre Vision, Mission and Values.
  22. Work in a manner consistent with the Health Centre's policy and commitment to Diversity, Equity and Inclusion in all processes and practices.
  23. Work in a manner that demonstrates self-reflection and personal accountability for work performance.

  25. Adhere to appropriate infection control practices including those related to Covid -19.
  26. Maintain status of fully vaccinated for Covid-19, including boosters as they become available - or exemption documentation if appropriate.
  27. Perform any other duties as assigned by your supervisor.

Accountability

This position is accountable to the Manager, Primary Care.

Qualifications

  1. Undergraduate degree in foods and nutrition from a recognized university or equivalent.
  2. Dietitians of Canada accredited Dietetic Internship or equivalent approved experience or graduate degree in community nutrition.
  3. Registered member in good standing with the College of Dietitians of Ontario.
  4. Experience in diabetes education and care, preferably having achieved and then maintain the Certified Diabetes Educator qualifications.
  5. Demonstrated knowledge of clinical/nutrition guidelines appropriate for clients.
  6. Three years nutritional counselling experience, preferably in a community-based setting.
  7. Experience in program development, implementation, monitoring and evaluation
  8. Proficiency in the use of a personal computer and software programs including Microsoft Office, email and internet.
  9. Demonstrated ability to communicate effectively in English, both written and verbally. Second language is an asset.
  10. Demonstrated ability to work collaboratively.
  11. Experience working effectively with culturally and economically diverse populations.
  12. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
  13. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
  14. The Health Centre requires that all staff be fully vaccinated for Covid-19 at the time of hire and maintain vaccination ongoing as boosters become available. Official proof of Covid-19 vaccination status is a mandatory requirement unless an exemption document is provided.
