    PHC is seeking an experienced Physician who has adult and pediatric urgent care or emergency room experience as well as and understanding of the primary care medical home model to join our welcoming and diverse community of providers. We are considering fulltime, part-time or per diem schedules.
    JOB SUMMARY:
    This provider will deliver patient-centered and culturally appropriate care to a diverse patient population and specifically manage patients’ acute conditions in both in-person and tele-health modalities including any urgent social and behavioral health concerns This provider will work closely with a collaborative team to ensure a proper bridge to the patient’s primary care medical home and avoidance of improper emergency room utilization.
    Responsibilities:
    • Provides triage, assessment, diagnoses, treatment of acute and episodic conditions and communicates with the local emergency departments, hospitals and outside providers when appropriate
    • Interprets on-site diagnostic testing (laboratory, x-ray, ECG etc) for diagnosis and management of patient conditions
    • Performs a variety of procedures necessary for urgent treatment of patients which include but are not limited to the following: incision and drainage, suturing, splinting, joint injection, wound care, gynecologic procedures. Point of Care ultrasound skills desired but not required.
    • Provides short-term management and bridging of care for chronic diseases (e.g., diabetes, high blood pressure) and short-term management and bridging of care for preventive care for adults and children including screening and immunization
    • Performs telemedicine, video and in person visits depending on the specific needs of the patient
    •  Communicates follow-up needs clearly with patient and patient’s primary care providers.
    • Cares for an average of 8-10 patients per four hour shift in a mix of modalities (tele, in person, video) on an open schedule utilizing OCHIN/EPIC HER
    • Counsels and educates patients in a culturally sensitive way on health behaviors, self-care skills, and treatment options.
    • Participates in quality activities as assigned by their clinical leadership.

    Qualifications:
    Education and Experience:
    • Must possess a valid California License issued by the California Medical Board and an MD/DO degree from an accredited medical school
    • Minimum 3 years of clinical experience. Interest in both in urgent care and primary care a must; preference for experience in urgent care or emergency department.
    • Basic electronic health record and computer skills required.
    • Experience treating both pediatric and adult patient populations (all ages) required.
    • Comfortable with minor procedures (sutures, casts, splints, etc.), and preliminary reads on EKGs and X-rays required.
    Licenses and Certifications:
    • Active Allopathic (MD) or Osteopathic (DO) Physician License with the State of California
    AND
    • Board Certified/ Eligible in Family Medicine, Emergency Medicine, Urgent Care Medicine, Emergency Medicine, Pediatrics or Med-Peds
    • Current Basic Life Support (BLS) Certification
    • Active Drug Enforcement Agency (DEA) Registration in good standing
    • Candidate must be able to meet PHC’s credentialing and privileging requirements successfully
    Required Skills and Abilities
    • Bilingual (Spanish/English) skills highly desired.
    • Excellent communication skills, both written and verbal.
    • Sensitive to the needs and situations of a multi-cultural population from a variety of income levels, dedication to work in a team
     
    Salary Scale: $227,000 to $275,065 per year  
    The compensation amounts listed above do not include relocation assistance, sign-on bonus, productivity or quality/service incentives, call pay, extra shift incentives, if applicable to the position.
