MDS Nurse RN – Skilled Nursing Facility

Facility: Metcalfe Nursing and Rehabilitation
Location: Edmonton, KY
Position Type: Part-Time

Position Summary

The MDS Nurse is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations and facility policies. This position works closely with the interdisciplinary team to ensure accurate assessments, appropriate care planning, regulatory compliance, and optimal reimbursement while supporting high-quality resident care in a deficiency-free skilled nursing facility.

Essential Duties and Responsibilities
  • Complete and coordinate Minimum Data Set (MDS) assessments accurately and within required timeframes.

  • Ensure compliance with CMS regulations, Medicare guidelines, and Kentucky state requirements.

  • Coordinate care plan meetings and collaborate with the interdisciplinary team to develop individualized resident care plans.

  • Review clinical documentation to support accurate coding and reimbursement.

  • Monitor resident status changes and initiate appropriate assessment updates.

  • Participate in Medicare and Managed Care documentation and utilization review processes.

  • Maintain accurate and timely records related to assessments and care plans.

  • Assist with survey readiness and support the facility’s deficiency-free standards.

  • Communicate effectively with physicians, nursing staff, therapy, social services, residents, and families.

  • Support quality assurance and performance improvement initiatives related to clinical outcomes and documentation.

Qualifications
  • Current RN or LPN license in the state of Kentucky.
