JOB TITLE:                              Residential Rehabilitation Counselor

EMPLOYMENT TYPE:            Part Time, 16 hours/wk

SCHEDULED HOURS:            Sunday and Monday 12pm-8pm

PROGRAM/LOCATION:        Northfield, Enfield, CT

ABOUT THE PROGRAM: Northfield

 Northfield is a Medicaid Rehab Option (MRO) residential treatment program for eight adults with serious mental illness which is staffed 24/7 located in Enfield. All residents are required to have Social Security and Husky C. Clients are referred by hospitals, the community and other CHR programs. The program focuses on skill building and teaching healthy habits through group and individual interactions. Group topics include nutritional counseling, money management, ADLs, interpersonal skills, medication management, coping skills, substance use education and leisure skills. Residential Rehabilitation Counselors and our therapy cat Lucy, partner with residents in their journey of acceptance of illness and learning the skills necessary to transition back into the community to live healthy independent lives. Northfield provides Hope to our folks and to their families.

DUTIES & RESPONSIBILITIES

  • Maintains physical environment and program standards to CHR, DMHAS, TJC, and DPH licensing standards.
  • Establish rapport and working relationship, utilizing person centered practices.
  • Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the daily events.
  • Teaches and assists all residents in areas of rehabilitation including nutrition and wellness, meal preparation, hygiene, laundry, banking and budgeting, basic home care skills, substance use education and recovery skills.
  • Assists/Manages administrative functions and tasks at the direction of the supervisor.
  • Prepares and facilitates educational and skill building groups and one to one intervention as directed by the treatment plan.
  • Monitors medications of all residents in accordance with the Adult Residential Medication policy and procedure.
  • Collaborates and coordinates care with providers and families as needed.
  • Assists residents in addressing medical and psychiatric needs.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, training, and other meetings as assigned.
  • Additional duties as required.

QUALIFICATIONS:

Education: Bachelor’s degree required; a minimum of 1 year relevant experience preferred.

Experience: Experience working with adults with Serious Mental Illness in a residential or supported living setting preferred; experience providing skills building teaching in group format preferred.

Licensure/Certification/Registration: CPR/First-Aid Certification within 6 months of hire or according to program requirements. Valid driver’s license required.

COMPENSATION*:               

Starting at $19.00/hr min., and up commensurate upon experience. 

*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
