Responsibilities
  • Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes - conduct patient home visits
  • Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's hospice needs
  • Manage emergency situations in a prompt, precise, and professional manner
  • Counsel patient and patient's family on end-of-life needs
  • Complete medication administration and treatment needs for all assigned hospice patients
  • Provide nursing care in accordance with applicable regulations and Company policy
  • Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications
  • Active RN state license
  • Valid drivers' license
  • Two or more years of applicable nursing experience
